Pre-Operative Instructions

  1. Call and make an appointment for your pre-operative history and physical.

  2. Ask the doctor if he/she will do the pre-operative blood tests.
     
  3. The surgery center or hospital will need a copy of your insurance forms or cards, and any other forms given to you by this office.
     
  4. DO NOT EAT OR DRINK after midnight the day of your surgery.
    If you
  5. normally take morning medications, you may take them with a sip of water on the morning of the surgery.
     
  6. Do not bring valuables to the hospital or surgery center.

    5.    Arrange for transportation to and from the surgery center. You
           will not be allowed
to drive yourself home after surgery.

    6.   It is not necessary for the person who is giving you a ride to the surgery to
           stay and wait at the hospital or surgery center.  The person may provide
           the staff with a cell phone or pager number which will be called just before
           you are ready to leave.

PLEASE DO NOT BE LATE FOR SURGERY!
